This is a Polish Legation certificate (Zaswiadczenie/Certificat) issued in Kaunas (Kowno), Lithuania, on 6 August 1940, confirming that Nachman-Icchok Sroka was a Polish citizen. It was issued to compensate for the lack of passport documents and to authorize travel abroad. Sroka, born 1 June 1915 in Warsaw, was single and worked as an office clerk (biuralista/employe de bureau).
A Sugihara transit visa stamp issued at the Japanese Consulate in Kaunas, Lithuania, dated 8 August 1940. It authorizes transit through Japan en route to Surinam, Curacao and other Netherlands colonies. The stamp bears the Japanese consular seal and handwritten Japanese notations indicating a limited transit period.

The first document this line has ever had, and it agrees with the register on four things independently. The register gives Sroka Nachman, 25, an office clerk, visa written 7 August 1940. The certificate gives Sroka Nachman-Icchok born 1 February 1915 — twenty-five that August — and records his trade as buchalter, employe de bureau, which is the same occupation in the two languages the form is printed in. His Polish certificate of citizenship, issued at Kaunas, with Sugihara's transit visa of 8 August 1940 for Curaçao by way of Japan, was offered at Winner's Auctions in Jerusalem on 29 November 2016 (auction 96, item 11) and did not sell.
